The office is open Monday through Thursday from 8:00 am to 12:00 pm and 1:00 pm to 5:00 pm. The office is open on Friday from 9:00 am to 12:00 pm and 1:00 pm to 5:00 pm. The office is closed Saturday and Sunday. Phone calls are forwarded to messaging at 4:00 pm daily. Messages left after 4:00 pm are answered the next business day in the order received.

Do I need a referral? If so, what do I need to bring with me to the first visit?
Patients do not need a referral for a new (acute) injury, but a referral is necessary for all chronic conditions. On your first visit, you will need to bring your most recent progress notes, diagnostics and operation notes (if applicable), demographics, insurance card, valid identification card, payment, and the referral. Your primary care physician can refer you if you do not already see a pain management specialist.
