What are your hours of operation?

The office is open Monday through Thursday from 8:00 am to 12:00 pm and 1:00 pm to 5:00 pm.  The office is open on Friday from 9:00 am to 12:00 pm and 1:00 pm to 5:00 pm.  The office is closed Saturday and Sunday.  Phone calls are forwarded to messaging at 4:00 pm daily.  Messages left after 4:00 pm are answered the next business day in the order received.

Do you have licensed therapists?

Yes.  All professionals are licensed and abide by their licensing boards’ codes of ethics and codes of conduct.

Is Dr. Schade affiliated with any hospitals? If so, which ones?

Do I need a referral? If so, what do I need to bring with me to the first visit?

Patient Referral

Patients do not need a referral for a new (acute) injury, but a referral is necessary for all chronic conditions.  On your first visit, you will need to bring your most recent progress notes, diagnostics and operation notes (if applicable), demographics, insurance card, valid identification card, payment, and the referral.  Your primary care physician can refer you if you do not already see a pain management specialist.

What insurances are accepted?

Are Letters of Protection (LOP) accepted?

Letters of Protection Acceptance

Yes, on a case by case basis.  Have your attorney contact us.

Is cash pay accepted?

Cash Pay Acceptance

We only see patients that have Medical Insurance.  Cash pay is accepted for co-pay payments and deductibles.
